Supplements can be ordered as follows (payment is required at the time of purchase):

• Call us at 859-277-5255 to ship your order via UPS.
• Email us at www.naturalchoicesdr@yahoo.com to ship your order via UPS.
• Directly from the company. Please call our office at 859-277-5255 to receive your activation code.

To order STANDARD PROCESS supplements directly, you must first register – select the PATIENT DIRECT option from the menu and enter the code Dr. Perkins gives you. Thereafter, you can login to select your products. Orders are shipped via UPS with a flat shipping charge of $10 per order. To access the website, click the following:  Standard Process. 

To order NP SCRIPT supplements directly, you must first register and enter the code Dr. Perkins gives you. Thereafter, you can login to select your products. Orders can be shipped via UPS or the US Post Office. To access the website, click the following: Np Script.
